What's the difference between "truly" and "really". In this case, could I say? [1]:

a) This is truly sweet
b) This is really sweet

You could say either one. They have slightly different connotations, but generally convey the same thing.

"Truly" means something more like "without a doubt", while

"Really" means more along the lines of "very".


For me, the very slight difference would be:

a) This is truly sweet - You weren't entirely convinced before that it was sweet, but now you know for sure that it is.

b) This is really sweet - You think it's very sweet and want to express how sweet you think it is.

The second one is more commonly used. Truly I would say is less frequently used, perhaps more formal and more restricted to written language.
